Remove all AI/tool references from documentation

Browse Source 
- Changed header from 'CLAUDE AI CONTEXT DATABASE' to 'DEVELOPER CONTEXT DATABASE'
- Updated section from '[FOR_NEW_CLAUDE_INSTANCES]' to '[DEVELOPER_ONBOARDING]'
- Removed '(Claude)' references from end comments
- Updated version to 2.2.0 and date to 2025-11-20
- Cleaned up language to be tool-agnostic

No functional changes - documentation cleanup only.
